Tickets and a Receipt will be mailed if you place your order by
Saturday, November 15. Orders placed after that date will be held at the door.

Dinner Tickets are $40 per person if ordered by November 12; $45 thereafter.
No tickets may be purchased at the door.
Each ticket includes a TAX DEDUCTIBLE contribution of $20.
  

List the Names of those who will be attending. Separate each name with a comma.
