You can purchase tickets online by using your Debit or Credit Card
over a Secure Server on the Internet.

Tickets and a Receipt will be mailed if you place your order by
Monday, October 24. Orders placed after that date will be held at the door.
No tickets may be purchased at the door.

Luncheon Tickets are $40 per person.
Each ticket includes a TAX DEDUCTIBLE contribution of $20.
  

List the Names of those who will be attending. Separate each name with a comma.

If you wish to be seated with an Honoree's group, please indicate which Honoree.
